Dean has been betrayed and hurt by everyone around him. Because of this, he can't act on his submissive tendencies which makes him extremely sick... The worst case Cas has ever seen.
The world thinks he's a lost cause, but Cas is willing to try and save his life. Only one problem: Dean is feisty and refuses to be helped. He fights and runs every chance he gets, hurting himself in the process. He has no reason to trust Cas, which makes his recovery unlikely. Though, maybe Cas can change that (:
Basically, Dean being sick and injured and Cas caring for him. Not always consensually tho. Hence 'Forced Care' tag. Then eventually they grow to love each other and its really sweet.
